Milson Island is situated 3 km northeast of Midden and Saltmarsh.

The 2,274-hectare reserve is situated in the northern edge of Sydney and lies between the suburb of Cowan to the south, and the Hawkesbury River to the north. Muogamarra Nature Reserve is situated 3½ km south of Midden and Saltmarsh.
